2022-23 Returning Member Assessment/Audition Registration Form

Hancock County Children's Choir leadership is busy planning our 2022-23 season, and we hope you will choose to join us for our 12th year of life skills mentoring and performing arts programming. Please visit HCCC's Audition page on our website for audition details, and complete the form below to register for an assessment (K-3rd fall 2022) or audition (4th-12th fall 2022). Following a successful assessent/audition, you will be invited to complete the Member Registration form to reserve your spot for the 2022-23 season. Questions? Email [email protected]!

Assessments/Audition Schedule and Location

Date: Saturday, April 23

Time: Click HERE to sign up for an assessment/audition time. We ask that you arrive 10 minutes early to sign in.

Location: Assessments/auditions will be held in-person at Mount Comfort United Methodist Church, 3179 North 600 West, Greenfield.

Mission: Hancock County Children's Choir is equipping youth with excellent musical education and essential life skills while igniting an interest in the arts in our rural community.

Vision: Hancock County Children's Choir envisions youth living fulfilled lives through strong character, confidence and perseverance and communities enriched through exposure to the performing arts.

WAIVER OF LIABILITY

2022-23 Season

I agree that I shall provide health insurance to cover any personal injury and property damage sustained by the student while participating in any activities of the Hancock County Children’s Choir (HCCC). The undersigned assume all responsibility for any and all risk of damage or injury that may occur to the student(s) named in this registration form during any activities associated with HCCC. Please be advised that there may be a higher risk of contracting COVID-19 and other respiratory illnesses by participating in singing and other HCCC activities.

Additionally, the undersigned hereby releases and discharges HCCC officers, directors, employees, agents, supervisors, instructors, and other HCCC participants from all claims,demands, rights or causes of actions present or future, whether known or anticipated and resulting from or arising out of or incident to the student(s) participation in the HCCC programs.
